Based on need, the Housing Support Worker works with youth to maintain permanent housing, navigate community services/resources and strengthen and/or create independent living skills. The Housing Support Worker provides transition planning and coordinates resources that wrap around the youth, supporting them in becoming self-sufficient both in their home and in their community.This Program assists youth between the ages of 16-24, living in the transitional housing building Ohana’s Landing.
